The Community
Services for the Elderly program (CSE) plans, funds, administers,
and subcontracts programs that provide services to frail low-income
elderly to assist them in maintaining independence at home. Programs
are designed to reduce reliance on institutional care, provide information
about resources in the community and provide cost effective, non-medical
services.

In-Home Services
In-House Specialist
The Onondaga County Department of Aging and Youth has an in-house
information and referral specialist. This full-time staff person
provides information, referrals and assistance to county residents,
linking callers with beneficial information regarding existing programs
and services.
Neighborhood Advisor Program
These outreach workers provide information and assistance to older
adults and their families. Advisors utilize key contacts to locate
older people who may not be in contact with service agencies. Advisors
match benefit programs with the older adult's financial and social
circumstances and help them apply to the appropriate programs. The program has been helping seniors in the community for over 20 years.
Care Manager
In cases where older adults have complex needs, a Care Manager can
work with the person and their family to address the individual's
situation and identify appropriate services.
Housing Options (HOME)
Housing Options & Management for the Elderly counsels seniors and
their friends or family regarding their housing situation. HOME
makes assessments based on the individual's needs and informs them
of available housing options.
Tenant Services
These services include information and assistance to senior tenants
and coordination of group activities at three housing facilities
operated by Syracuse Housing Authority.
Social Work Services
Aurora, Inc. operates a Social Work Services Program for older persons
who are vision and hearing impaired. Social workers counsel and
provide information and connections to assistive equipment.
Senior Centers
These centers offer inclusive and diverse programming designed to promote physical and emotional wellness.
Social Day Programs
The Salvation Army operates a Social Day Program at the Adult Community
Center in downtown Syracuse. Open five days a week, the Day Program
offers group social activities and congregate dining.
Transportation
The Department of Aging and Youth provides funding for transportation
services to residents of Onondaga County through the following programs:
CNYRTA Centro/Call-A-Bus; Catholic Charities Salina Civic Center
Van; Syracuse Northeast Community Center; The Salvation Army Transportation
Program; The Baldwinsville Senior Express, and the Native American
Outreach Program through the Bob Cecile Center and the United Methodist
Church Conference of CNY.
Elder Abuse
Elder Abuse The Elder Abuse Community Education Program run by Vera
House offers presentations to community groups and aging services
providers on elder abuse, how to properly identify warning signs,
and protocols for reporting suspected cases.
Project REACH
Project Reach is a community partnership designed to identify at-risk
older adults. Referrals to Project Reach are made to Onondaga County
Department of Aging and Youth. The confidential referral is forwarded
to an outreach worker who visits the home and assesses the situation.
Long Term Ombudsman Program
The Long Term Care Ombudsman Program, run by the InterReligious
Council of Central New York, recruits, trains, and certifies volunteers
to advocate for residents in county long-term care facilities.
Legal Services
Through its Older Americans Unit, Legal Services of CNY, Inc. provides
legal assistance to residents of Onondaga County who are 60 years
of age or older. Areas of assistance include: entitlement issues,
utility problems, wills, health care proxies, living wills, power
of attorney, and consumer debt issues.
Short Term Respite
The Short Term Respite Program, run by Home Aides of Central New
York, Inc., provides up to four hours per week of respite to clients
or caregivers 60 years of age or older.
Telephone Lifeline
Volunteers callers provide daily contact, reassurance and conversation
to the disabled and elderly homebound. There is a protocol to follow
if there is no response to that call.
Project Fix
This program contracted to Catholic Charities assists seniors with
minor home repairs.
