Children with Special Health Care Needs (CSHCN) are those children (0-21 years) who have or are suspected of having a chronic physical, behavioral, emotional or developmental condition, which requires health and related services. The goal of this program is to provide families who have Children with Special Health Care Needs with information and referrals to access health services and community resources.

The Children with Special Health Care Needs Program can help families access the following:
• The Physically Handicapped Children's Program
• Specialty care providers and related health care services.
• Health insurance, such as Child Health Plus (CHP) and Medicaid.
• Community resources for family support and services.
• Information on medical home coordination of care.
