Persons eligible to get certified copies of Birth Certificates
are those listed on the Birth Certificate (Mother, Father or Child).
1.
You
may fill out a Request
for a Birth Certificate form or obtain an application at the office of
Vital Statistics.
2.
If filling
out online form, please complete form and sign. Bring the application
and a signed form of identification to: Office of Vital Statistics, 421
Montgomery Street, Room 20 Basement Civic Center, Syracuse, NY 13202

of acceptable identification are:

4.
If
paying by credit card, the name of the credit cardholder must match the
applicant’s name and identification.
Please note: A Birth Certificate will not be issued
unless a signed form of identification and payment are presented with a
completed request form.
