A person may get a death record if they are the spouse, the
child* or the parent of the deceased. If you are not one of the above listed,
a letter or document on letterhead from the office or the agency that needs
the death certificate for a legal purpose must be faxed with your
identification.

Complete
online form Request for a
Death Certificate. (Secure Server) Incomplete applications may cause a delay in processing.
2.
The
credit cardholder name must match the applicant name on the Death Certificate
Request Form and the name on the faxed identification.
3.
A
copy of your signed identification must be faxed. Examples of acceptable
identification are:

Social Services Card with signature
4.
If
the identification you are faxing is two-sided, both sides must be faxed.
5.
Fax
copies of identification to (315) 435-3614 using the fax cover sheet provided
with your order acceptance.
Please note: A Death Certificate will not be issued
until a copy of your identification is received. Please make sure the copy is
readable. Enlarging image on copier may help legibility. Please sign below image before faxing
it.
* A child of the deceased must provide a copy of their own
birth certificate showing deceased parent listed as proof of relationship.
