A person may get a death record if they are the spouse, the
child*, or the parent of the deceased. If you are not one of the above
listed, a letter or document on letterhead from the office or the agency that
needs the death certificate for a legal purpose must be submitted with the
request.

may fill out a Request
for a Death Certificate form or obtain an application at the
office of Vital Statistics.

If
filling out an online form, please complete form and sign. Bring the
application and a signed form of identification to: Office of Vital
Statistics, 421 Montgomery Street, Room 20 Basement Civic Center, Syracuse,
NY 13202

If
paying by credit card, the name of the credit card holder must match the
applicant’s name and identification.
Please note: A Death Certificate will not be issued
unless a signed form of identification and payment are presented with a
completed request.
*A child of the deceased must also provide a copy of their own
birth certificate showing deceased parent listed as proof of relationship.
