A person may get a death record if they are the spouse, the
child*, or the parent of the deceased. If you are not one of the above
listed, a letter or document on letterhead from the office or the agency that
needs the death certificate for a legal purpose must be faxed with your
identification.

Please
fill out a Request
for a Death Certificate form and have credit card information ready
prior to phoning in order.
2.
Credit
card information must include cardholders billing address. The credit
cardholders name must match the applicant name and the faxed identification.

Call
in request at (315) 435-3241.

A
copy of your signed identification must be faxed. Examples of acceptable
identification are:

If
the identification you are faxing is two-sided, both sides must be faxed.
6.
Fax
copy of identification to (315) 435-3614.
Please note: A Death Certificate will not be issued
unless a completed request form, copy of your identification and payment
information is provided. Please make sure copy is readable. Enlarging image on copier may help
legibility.
*A child of the deceased must also provide a copy of their own
birth certificate showing deceased parent listed as proof of relationship.
