Q. Can I get a Birth Certificate for someone else?
A
. Yes, but it must be done in person and you must have a letter from them authorizing you to do so along with a copy of their identification.

Q. If I am the legal guardian of a person, how can I get their Birth Certificate?
A.
If a Birth Certificate is requested by the legal guardian of a person, he or she must show court certified legal guardianship papers that have been date stamped within the last year.

Q. If I was adopted, will my Birth Certificate be on file?
A.
A person who was born in Onondaga County, and legally adopted, can obtain a Birth Certificate with their adoptive parents names. If a Birth Certificate was never sent to us at the time of the adoption from the New York State Department of Vital Records, we can obtain one from them. For more information on adoptions, contact the Adoption and Medical Information Registry at (518) 474-9600.

Q. Can I get my Birth Certificate if I am under 18?
A.
Yes, but you must have identification and it will be a short form without parental information.
