The Facilitated Enrollment Program offers families in Onondaga County the opportunity to enroll in the Public Health Insurance program that will meet their medical and dental needs. Facilitated Enrollment offers an easy way for families to apply for Family Health Plus, Medicaid and Child Health Plus. The same application can also be used to apply for WIC and PCAP. Facilitated enrollers will meet with families at convenient times and locations to complete an application and gather the necessary documents to determine their eligibility for health insurance. Enrollers are community-based at three locations within the city and county.
