   Peer Engagement Referral Form

What is the Peer Engagement Program?


In conjunction with Coordinated Care Services Inc. (CCSI) Onondaga County Mental Health provides Peer Engagement Services. CNY-PREP is a voluntary program funded by the Office of Addiction Services and Supports (OASAS) that provides supports for those individuals and families who are struggling with substance use issues.

 

What does a Peer do?

 

Peers with their own lived experiences provide a unique understanding of the challenges faced by those seeking wellness and recovery.

A Peer will walk with you and provide support in the development of a recovery plan. Peers approach recovery with an understanding that each person follows their own path. Peers embrace the uniqueness of each individual’s ability to create a life of meaning and purpose as they define it for themselves.

 Peer supports are voluntary

 Peers share hope, support, and encouragement

 Peers offer one-to-one relationships

 Peers can connect people to a wide range of resources and supports that promote wellness/recovery

 Peers believe that health, home, purpose, and community are elements that lead to lasting wellness and recovery
