The program partners with Upstate University Hospital and Upstate University Hospital Community Campus to reach those in treatment for substance use, either in the Emergency Department or in-patient admissions. The hospitals make most of the referrals to the Peer Engagement Program.  The initial contact is made at the hosptial and then follow-up is conducted post discharge by the Peer Engagement Specialist.

The Peer Engagement Program also supports the Opioid Bridge Clinic at Upstate University Hospital. Peer Engagement Specialists join their patients at their appointments to ensure that they are connected to treatment or services that best meet their needs.

What Does a Peer Engagement Specialist Do?

  • Provides peer support and advocacy for people who experienced a drug overdose in a medical setting.
  • Works with hospital social workers and discharge staff to facilitate substance use treatment, support and recovery services.
  • Builds relationships with patients who frequently use the ER and/or have a dual diagnosis.
  • Identify personal obstacles that have prevented previous recovery efforts from being successful.
  • Empowers people to make informed choices about their treatment and/or support options.

What is the Process to Utilize this Service?

  • The Hospital Social Worker/Discharge Planner obtains patient’s consent to receive our service and makes referrals to the program.
