Ordering Birth Certificates In Person

Payment may be made by Cash, Check, Money Order or Credit Card (VISA or Mastercard). The fee is $30.00 for the first certified copy and $15.00 for each additional copy of the same certificate when ordered at the same time.
Certificates are issued at the time of request.
Persons eligible to get certified copies of Birth Certificates are those listed on the Birth Certificate (Mother, Father or Child).

• 1. You may fill out a Request for a Birth Certificate form or obtain an application at the office of Vital Statistics.
  2. If filling out online form, please complete form and sign. Bring the application and a signed form of identification to: Office of Vital Statistics, 421 Montgomery Street, Room 20 Basement Civic Center, Syracuse, NY 13202
  3. Examples of acceptable identification are:
      • Photo Driver License
      • Non-Driver Photo License
      • Passport
      • Military ID with signature
      • Employer's Photo ID with signature
      • Photo Social Services Card with signature
  4. If paying by credit card, the name of the credit cardholder must match the applicant’s name and identification

Please note: A Birth Certificate will not be issued unless a signed form of identification and payment are presented with a completed request form.
