Our Mission

The Department of Long Term Care is composed of two Divisions; the Extended Care Division, Van Duyn Home and Hospital, and the Community Services Division & Resource Center.

VAN DUYN HOME AND HOSPITAL

Van Duyn Home and Hospital is a County owned and operated skilled nursing facility. The mission of Van Duyn is to provide its residents with the highest quality care in a warm, comfortable and caring environment, responsive to their medical and social needs, potential and lifestyle, and to maximize the quality of life of each resident.

COMMUNITY SERVICES DIVISION & NY CONNECTS
The mission of the Onondaga County Long Term Care Resource Center is:

To promote and provide access to appropriate and cost effective long term care for all residents of Onondaga County;

contain the cost of long term care through effective case management for individuals receiving publicly funded long term care;

support other individuals in need of long term care to enable them to utilize private resources in the most effective manner;

to foster the development of programs and plans responsive to a changing demographic and public policy environment;

to monitor compliance with State and Federal regulations in programs for which it is responsible.

To help fulfill this mission the Resource Center additionally runs NY Connects Onondaga, offering county residents long term care information, assistance and screening services. NY Connects services can be accessed at (315) 435-1400.
